A note from our pharmacy provider, VRx, please plan accordingly:

There has been a major delay in the creation and FDA release of the Flumist. As of today 9/17/15, only 7 different lots have been released. A “lot” is like a batch of the vaccine created and released at the same time. This same time last year, 34 lots had been released. That is a significant shortage. We are hopeful that we will received the Flumist vaccines that we have ordered, but because of the shortage, there is a high likelihood that we may not have it for those anticipating to receive it.

  • You must register for yourself and ALL dependents receiving a flu shot at the Clinic
  • Bring your Blue Cross Blue Shield (BCBS) insurance card to the Flu Shot Clinic
  • Please download and fill out the following forms and bring them with you to the Flu Shot Clinic:

If you are unable to come to one of the USU Flu Shot Clinics, you are welcome to go to a pharmacy within the BCBS network. When visiting a pharmacy be sure to show your BCBS insurance card.
